Trinity Human Resources: Employee Resources

Human Resources Forms

Many employee forms/documents may be found on ADP Workforce Now including pay statements, W-2s and benefit program details.

To get your ADP online account up and running, please follow these steps.

  1. From a desktop or laptop computer, go to ADP Workforce Now to create your account. Click the “Register Now” link to get started.
  2. When asked for a Registration Code, please use the following: TWU1897  If this does not work, please try TWU1897-ADP
  3. Follow the prompts which will ask for your name, last 4 digits of your SSN and your birth month and day. If you get an error here, please contact HR as there may be incorrect information in the system that we need to update.
  4. Once you have completed the set-up process please log out of ADP.
  5. Then log back in using the workforcenow.adp.com link and you should see a number of options under the Myself tab in the main toolbar. You will also find a wealth of benefit information under the Resources > Company Information > Tools/Reference page.
  6. Please review your address and contact information under the Myself tab.  Please add a non-Trinity email address if one is not already included.

You may access these commonly used forms by clicking the links below. Once you submit a form, it will be automatically routed for processing.

Tax forms should be completed, printed and dropped off at the HR office MH 134. Given the sensitive information contained on these forms we do not recommend emailing them. If you do not have a printer, we have hard copies available to you in the office.
